Ability to add new locations in the app if they are not in the Rolodex

For fields such as "Location of Death", "Location of Remains", etc. We need the ability to add a new location without having to select "Other". There needs to be a field to type in a new location. Currently if you choose "Nursing Home" for example, you only have access to what is currently in the funeral homes rolodex.
