Pull Funeral Homes from Rolodex in Ship To/From Dropdowns Merged

On the Shipping Information page, if you choose Facility Type "Funeral Home" the only options are the existing funeral home you are logged in to. We would like for this to allow us to choose from any Funeral Home we have listed in our Rolodex.
