Add Receipt Number to Insurance Assignments (and populate on System Report)

There is no receipt number field on Insurance Assignments, but there is a column for it in the Payments & Adjustments System Report.

I think it would be beneficial to be able to log a receipt number in an insurance assignment, at least after the funds have been received, if nothing else.
