Survey Report Column Additions Merged

Would like to have these options to add as Survey Report columns: Case Tags, Disposition Date, Case Origin, Decedent Race, Custom Fields. I am currently running an additional report to supplement my survey report and include this information. I would really like to have this to be survey report column options so I can run a single report.
