A way for families to initiate Imminent Need case from our website

I am reworking our website "Planning" area as we are adding the Arrangement Guide, and it occurs to me that allowing families to initiate an imminent need case for their loved one from our website would be a very useful tool. They could even use it after a death occurs if it happens to be in the middle of the night at the hospital or something, and we won't be notified until morning. We could then make it an "at need" case.

If our landing page had one button that said "Planning for the Future" that went to Arrangement Guide and a button that said "Planning for death that is near or has occurred" that went to the Planning Center, that would have a lot of value for us and our families.

Often, family members are awake at odd hours, looking to "do" something, and it would be helpful to us if this is what they did.
