Add city, state next to rolodex entries in dropdowns for venues

There are often multiple rolodex entries for places with the same name (there are 6 DIFFERENT St. Mary's Churches in the areas surrounding Cleveland) when scheduling a service and choosing a venue, it would be nice to see the city, state in the dropdown so they can easily identify which venue they need to be selecting
